GallBladder Stones & CBD Stones

GallBladder is a pear-shaped accessory digestive organ tucked under the liver in the right upper abdomen. The function of the Gall Bladder is primarily storing the bile juice which is produced by the liver; when we eat fatty food the gall bladder squeezes this bile through the common bile duct into the intestine. When cholesterol or fat concentration increases in the bile juice, the juice precipitates as stone in the Gall Bladder. It can occur in all age groups and in both sexes, though more commonly in females.

Common symptoms of gallbladder stone disease or cholelithiasis include persistent pain in the upper belly and back that lasts for several hours, accompanied by nausea, vomiting, indigestion, bloating, and heartburn.

GALL BLADDER STONE TREATMENT

Treatment for gallbladder stones often involves surgical removal of the gallbladder, a procedure known as cholecystectomy, which is the most effective way to eliminate gallstones and reduce the risk of recurrence. While conventional surgery involved making an incision in the right upper abdomen, the current approach utilizes minimally invasive techniques such as laparoscopy. Laparoscopic cholecystectomy, considered the gold standard treatment for gallstones, is a safe procedure performed under general anesthesia and involves only four small incisions. It can also be conducted using a Single-Incision Laparoscopic Surgery (SILS) technique, further minimizing the impact on the patient.

CBD STONE TREATMENT

CBD stone treatment addresses stones found within the bile duct, which are typically formed within the gallbladder but may also develop in the bile ducts following gallbladder removal. These stones can migrate into the bile duct due to gallbladder contractions, leading to symptoms such as upper abdominal or back pain, jaundice, vomiting, and fever. Prompt removal of these stones is crucial in most cases.

Traditionally, open CBD exploration was the preferred method for removing these stones. However, advancements in medical techniques have led to the widespread adoption of endoscopic retrograde cholangiopancreatography (ERCP), a minimally invasive method for removing CBD stones in a day care setting without leaving any visible scars. Early removal of the gallbladder via laparoscopic surgery is often recommended following CBD clearance through ERCP to prevent the reoccurrence of stones in the CBD.

In cases involving large stones or challenging anatomical conditions, endoscopic clearance of the CBD may not be feasible. In such instances, laparoscopic CBD exploration and clearance become the procedure of choice, allowing for both CBD clearance and gallbladder removal to be performed in a single sitting. This approach streamlines the process, reducing the need for two separate procedures and resulting in a shorter hospital stay for the patient.
